- A transfusion must be completed within 4 hours from the time that the blood product is issued from the blood bank. Any transfusion that has not been completed within the 4 hour time limit must be discontinued. (PC 15-05, Blood component Therapy, pg 7).
- When the blood product is received on the floor, a separate paper secured to the product will give the blood bank release time.
- All blood products will be sent to the unit through the tube system in a “secure tube”. The number for the secure tube will always be 9657. Secure tube for blood products will start at 0800 on July 28th.
- The white unit tag attached to the blood must remain attached until the blood is completed. Once the unit tag information is documented in EPIC the unit tag can be discarded. NOTE: The unit tag MUST remain on the blood unit until completion.
- In the documentation flow sheet the end time of the transfusion must be documented as “Completed “.

RNs, ERTs, and Clerks will be receiving a competency this week that they should complete on the handling of blood products from the blood bank. This is in response to a Joint Commission mandate after the June survey of the laboratory. The supervisors will be passing this competency out - only Bonnie or one of the supervisors will be able to sign you off.
The completed competency needs to be returned to Bonnie by August 1st!

The second part of the Joint Commission mandate to lab was accurate reporting of critical lab values. Our practice of using pink sheets to document the critical labs and giving to the medical staff has not changed. This is your answer for Joint Commission when they come to review us.
