Traumatic arrest
Airway/respiratory compromise
Flail chest/major chest injury
Sustained BP < 90
GSW and stab wounds to torso (neck to genitalia)
Penetrating injury to head and face (major)
Depressed skull fracture
GCS < 14
Focal deficits (unilateral pupil, paralysis)
Abdominal finding suggestive of patient needing surgery
Major extremity vascular injury
2 or more proximal long bone fractures (i.e. femur, humerus)
Amputation proximal to wrist and ankle
Severe hypothermia - < 32.1 C (< 89.8 F)
Severe burn patient with minimal or no prior notification
EMS judgment
MD judgment
